Gone are the days when dental treatments more over dental implant ‘s were feared for pain. Now age is no barrier in getting fixed teeth with dental implant. One such inspiring story is of this 92 year old patient who visit Royal Dental Clinics with her son. Her concern was that she was missing teeth, and on weekends when on family lunches, she felt very awkward to sit in a corner and sip on soup, while others enjoyed a hearty meal. She only asked one thing to Dr Chirag Chamria; “Can you give me teeth so that I can eat with family?”.
Being experienced in geriatric dentistry, Dr Chamria decided to place quad zygomatic dental implant in upper jaw and regular dental implants in the lower jaw. Also, decided to place SAPTeeth with PEEK framework. The case was posted for treatment next day. During the day, the team at Royal Dental Clinics, prepared the surgical guides with the help of virtual 3D planning and the patient was deemed fit by the physician.
The Challenge
The challenge in case planning for geriatric patients are multifold. From thin or atrophic bone quality to multiple medical issues such as heart disease, stroke, diabetes etc. And, finally the age-related fatigue and tiredness. The placement of dental implant required meticulous planning and expertise. Process for Fixed Teeth with Dental Implant
The morning of treatment, patient was nervous but excited to be getting her new set of fixed teeth. The patient’s medical history warranted a thorough checkup and care. Dr Chirag Chamria, being an oral surgeon with an excellent in-house team of experts, the implants were placed in both the upper and lower jaw in about 90 minutes. As the surgical guides were already prepared, the whole procedure was bloodless and virtually cut free. After this the measurement was made using an 3D camera called as intraoral scanner.
After about 4 hours of implant placement and measurement, the laboratory kicked into gear. The designing to fabrication to finalization of the fixed teeth was done. During this time, the patient with her son were resting in the suite rooms. The final SAPTeeth with PEEK framework was fixed in late afternoon for the patient. After which, patient was asked to chew on a piece of biscuit to check for occlusal comfort. Patient was slightly tired but chewed on comfortably.
At one week, the patient and her son were happy they decided to get this done. They really didn’t believe at first that the process could be done so smoothly.
Misfortune
However, misfortune stuck. After about 6 months of treatmetn, the son one day calledup the clinic and narrated an incident which has occured. Patient was diagnosed with a clot in her brain and an emergency brain surgery was performed. Amidst that surgery, a tube was placed though the mouth. Probably during the placement or removal of the tube, the upper bridge and one of the implants were severely damaged. At this stage, the patient was left with no teeth in the upper jaw again. But the ray of hope was that the lower teeth were intact.
Ray of Hope
After about 6 months, the patient’s medical condition settled, and now the process to give her teeth back could be stated. Following even more safety, the fractured implant was replaced and a new bridge was fabricated with the same process. The new teeth were given and patient is now comfortable in chewing almost anything from breads to biscuits.
