# Assessment of Mouth Ulcers and Oral Lesions

A mouth ulcer, patch or lump should be assessed if it persists, changes or causes concern. Tell the clinician how long it has been present, whether it is getting larger or bleeding and whether similar problems have occurred before. A painless change can still deserve examination.

An ulcer lasting longer than three weeks needs a dentist or doctor’s assessment. Seek advice sooner if a lesion is worsening or you have other concerning symptoms. Examination may lead to advice, review or further investigation, including a biopsy where indicated. The appearance alone does not establish the diagnosis.

## Let’s understand what you need.

Your consultation is an opportunity to understand the findings and compare suitable options. Ask what can happen on the first treatment day, which later visits may be needed, and what the estimate includes. A family member can help you consider travel, recovery and maintenance arrangements.

## Tell us what changed

Describe when the lesion appeared, whether it is growing or recurring, and any pain, bleeding or difficulty swallowing. The dentist reviews trauma, dentures, habits, medicines and medical history. A photograph can help record a change, but it cannot rule out a serious cause.

## When to arrange prompt assessment

Seek assessment for a persistent ulcer, a red or white patch, a lump, unexplained bleeding or difficulty swallowing. Earlier review may be appropriate for worsening symptoms. Tobacco and alcohol history are relevant, but a suspicious lesion needs assessment even without these risk factors.

## Examination and further tests

The clinician examines the lesion and surrounding tissues. Management may involve removing a source of irritation, reassessment, investigation or biopsy where indicated. A biopsy provides tissue for diagnosis; the need and method are explained before the procedure.

### What helps the clinician assess a lesion

Mention tobacco or other oral habits, medicines, recent dental work and any possible irritation. If the appearance has changed, dated photographs may help describe the history. The clinician still needs to examine the area and surrounding tissues. Explain swallowing, speech or other functional changes rather than focusing only on the colour of the lesion.

### Understanding a biopsy recommendation

A biopsy takes a tissue sample for examination when it is needed to clarify a finding. Ask why it is recommended, how the procedure will be arranged and how results will be discussed. A recommendation for biopsy is an investigation step and does not by itself mean a cancer diagnosis.

Questions before you decide

## Your questions answered

Does every ulcer need a biopsy?

No. The examination, duration and features determine whether biopsy or another investigation is required.

Can a painless patch be important?

Yes. Some significant lesions are painless. Persistent red or white patches and lumps need assessment.

Should I wait three weeks if it is worsening?

No. A worsening, bleeding or enlarging lesion should be assessed sooner. The three-week threshold is not a reason to delay concerning symptoms.

What should I tell the clinician about a mouth lesion?

Explain when it started, changes in size or colour, discomfort, bleeding and any tobacco or areca-nut use. Mention medicines, medical conditions and similar earlier episodes.

What happens if a biopsy is recommended?

The clinician explains the reason, sampling procedure, consent and how results will be discussed. Further treatment depends on the examination and pathology findings.

### Treatment follows the diagnosis

Not every lesion is cancerous, and many ulcers have benign causes. Equally, repeated home treatment should not delay the assessment of a persistent lesion. The plan and estimate reflect the examination, investigation and treatment required. Specialist referral may be needed.

### Follow the review plan

Attend reassessment even if discomfort improves when the clinician has asked to review the lesion. Keep a record of changes and contact the team if it grows, bleeds or affects eating. Breathing or swallowing difficulty requires urgent medical attention.

### Oral-surgical assessment and rehabilitation

Royal Dental Clinics provides oral-surgical care and rehabilitation after oral cancer treatment. Restoring speech, chewing, appearance or separation of an oral defect requires an understanding of the original disease, surgery, tissue condition and ongoing medical care.

For a new lesion, the priority is assessment and appropriate diagnosis rather than cosmetic or implant planning. Bring pathology and oncology records where relevant. A rehabilitation plan may involve different clinicians and can include removable or fixed prosthetic options according to the findings.
